Prairie Mental Health Services are available by appointment only.
Scheduling an initial appointment begins with calling our office at 660-747-4700. You will be asked to provide some personal information: the name of the patient, address, date of birth, social security number, insurance information, and e-mail address. We will verify your insurance benefits and eligibility; please have your insurance card available at the time of your call. If no one is available to answer your call, please leave a short message and we will return your call in a timely manner.
Within 3-4 days of your initial phone call, you will be receiving a packet of information and forms to by mail to complete, sign. You will bring these completed forms and assessment with you to your initial appointment. Completing these forms is essential to completing the initial psychiatric assessment. Your initial appointment will take 2 to 2 1/2 hours; all follow up appointments will be either 30 or 60 minutes in length.
We expect you to call 24 hours prior to canceling an appointment; we understand that individuals encounter unplanned emergencies and unforeseen events, in these circumstances please call as soon as possible.
